- Disability certificate in case of illness, circumstance leave (family), sports certificate, health certificate for work,  breastfeeding certificate,…

Prescriptions and certificates cannot be obtained by telephone. The RIZIV no longer allows us to provide this after a telephone request. Therefore, always ask for sufficient prescriptions when you come for a consultation. 

 

Completing forms requires information and time. That is why you should also come for consultation for this.
